This information is being collected by the authors of the letter to the Times, 28th May.

The main purpose of this questionnaire is to enable us to continue pressure on Colleges, PMETB, BMA, DoH to recognise flaws in the current round of appointments, and its fallout, and to help represent your views in discussions with the Tooke Review.

Because responses may differ according to the outcome of recent applications, and according to different grades, specialties and locations, we need some initial information about you.

You will be asked at the end of the questionnaire for your permission(s) how we might use your answers.
